Jazz chords : Comments and Ideas

I often use C6 as an ending chord (see attachment).

By the way I have modified some of the chords in the song Autumn Leaves in the key of G to the best of my knowledge.I would like to have your opinion.(See attachment)

I love to substitute chords that fits the song.I don't know how, somehow it comes naturally by sound, trial and error.
